At the conclusion of the
Health Care for the Homeless is ONTRACK with Recovery conference, participants will be able to:
•
identify the health disparities and the unique challenges and barriers that homeless persons encounter on their road to recovery;
•
explain the concept of recovery and the importance of promoting recovery-oriented practices for persons experiencing homelessness;
•
describe the evidenced-based and promising practices of Screening, Brief Interventions and Referral to Treatment (SBIRT), integration and how to apply concepts into their own service areas and recovery; and
•
assist individuals in using their unique skills and strengths in their recovery journey.
Who Should Attend:
This conference is designed for family members and persons that have experienced homelessness and community-based service providers such as physicians, psychiatrists, social workers, nurses, case managers, service coordinators, administrators and others interested in the health needs of persons experiencing homelessness.
